Bloomington Extreme Donation Request

• All requests must be submitted, in writing, on your organizations letterhead, via mail or fax.  NOTE: Submitting a request does not guarantee a donation.
  • Organizations must be non-profit, including schools and churches.
• All requests must be accompanied by a COMPLETED Donation Request Form. Forms submitted that are incomplete will not be considered.  
• Requests for fundraisers to benefit an individual or family are generally not honored.
• All requests must be received 4 weeks in advance.  
• Organizations will be granted one request each year, so that we can attempt to help as many organizations as possible.
• Requests for monetary donations will NOT be accepted.
